1. Recipients name, current address & phone number, and e-mail address (if available).
2. Recipients Date of Birth and Social Security Number.
3. Name and Address of College (Specifically where funds should be sent).
4. Proof of Enrollment from the College.

NOTE: Scholarship money can only be sent directly to the college, it cannot be sent to an individual (including the recipient).
